Avon and Wiltshire Partnership Trust reduced the number of out-of-area placements for patients receiving mental health services, to ensure that all patients across its large footprint could receive care close to home.

What the organisation faced

Avon and Wiltshire Partnership Trust delivers a range of mental health services, including inpatient services for adults and secure services. These provide support and treatment and aim to help patients become well enough to return to living in their local community.

The trust covers two ICSs across 1,100 square miles, serving a population of around 1.8 million across Bath and North East Somerset (BaNES), Swindon and Wiltshire, Bristol, North Somerset, and South Gloucestershire.

This large footprint poses a challenge as it contains both urban and rural geographies, and it means that services are spread out. The location of the trust’s own services, particularly those for inpatient care, mean that patients may be placed far away from home.

However, in some cases, patients were being placed outside of the trust’s footprint, including some hundreds of miles away. By being placed in an unfamiliar area, this added to the distress that patients were already facing and made it more difficult for their loved ones to visit.

As well as being more difficult for patients and their families, data showed that patients placed outside of the trust’s area also had longer-than-average lengths of stay, indicating poorer outcomes, with discharge planning and continuity of care also being more difficult.

Finally, even for patients placed within the trust’s area, some of these were with different providers, including those from the independent sector, which in this particular case made delivering care less efficient.

What the organisation did

The trust wanted to reduce the number of out-of-area placements and sustain that reduction.

In early 2020, an initial piece of diagnostic work was carried out to better understand the flow of patients. They found that frameworks and policy guidance that were in place to improve patient flow needed additional focus in order for them to be implemented routinely.

The trust held a rapid improvement phase to see what difference could be made, covering 44 wards including 11 acute wards.

Some key actions and areas of focus included:

A strong focus in reducing admissions was vital, and that involved working with the understanding that community treatment was the preferred option, as well as being clear about always defining the reason for admission and the intended outcomes.

Resolving out-of-area placements meant liaising with regional partners and the trust met with neighbouring trusts and networks to share knowledge and learning, including Devon NHS Partnership Trust, Solent NHS Trust and the South West Strategic Clinical Network.

Results and benefits

There has been a significant reduction in out-of-area placements from a peak of 68 patients in 2020, and in December 2023 the trust had no patient in an out-of-area placement outside of their geographical boundary.

James Eldred, clinical director for right care, said at the time: “This was a proud moment for the trust. Everyone I worked with understood that admitting patients to beds around the country was something that needed to change, but actually delivering that change has been a massive challenge and a very long process. We take confidence from knowing that none of our patients will be admitted to wards far away from their homes. It proves that we can make a huge difference to peoples’ lives even when resources are tight, and the challenge is great.”

The trust is now aiming to eliminate the use of non-trust beds.

The group saw total revenue reach £942 billion (BRL 6.1 billion), up 1.1% in constant currency but down 5.7% on a reported basis.

Adjusted EBITDA totalled £105 million (BRL 683 million), with an 11.2% margin, up by 110 basis points and representing another quarter of "solid profitability expansion" for Natura & Co.

However, the company reported a net loss of £144 million (BRL 935 million), compared to a net loss of £100 million (BRL 652 million) the previous year, impacted by discontinued operations, higher taxes from country mix and FX losses and hyperinflation accounting impacts.

Net revenue at Avon International decreased 4.7% in constant currency, impacted by the continuation of weaker promotional execution as mentioned by the company in the previous quarter.

From a category perspective, beauty was also impacted by promotional execution and dropped 4.2%, with the exception of fragrance, while home and style was down 22% amid a planned portfolio reduction.

In an effort to drive growth, Avon International said it is actively exploring opportunities from other distribution channels, including retailers. It is already being sold in the UK via Superdrug, in Italy via Naima stores and in Turkey via representative's retail franchise stores.

In addition, efforts to strengthen gross margin and streamline operations led to only a "slight" decrease in adjusted EBITDA margin.

Fabio Barbosa, CEO of Natura & Co, said: "Avon International had a slow start of the year, following solid Q4 2023 profitability performance.

"The new management team took office in January and is working on simplifying the market, focusing on key countries, and enhancing our portfolio with superior promotional execution. We believe these steps are crucial to stabilise revenues and keep us on track to improve profitability.

"We are also continuing to study a possible separation of Avon and Natura, as we announced in February, in line with our goal of simplifying our corporate structure and giving more autonomy to the business units. We will inform the market as soon as we have news on this subject."

News emerged earlier this year that Natura & Co. is assessing the possible separation of Avon International into an independent publicly traded company as it attempts to unlock further shareholder value.

The structure of the potential transaction is still being considered, but it was previously expected that it would result in two separate, standalone entities (Natura and Avon) with "unique business plans, independent governance and management teams better equipped to pursue more tailored strategies to drive long-term shareholder value".

First Bus service to link Trowbridge, Bradford on Avon and Bath

A new bus service is to be launched in April to provide faster connections between Trowbridge and Bradford on Avon and Bath.

The new D1x service will begin from Sunday, April 7 and will provide a more direct connection into and out of Bath.

It will use the A363 via the Bradford Road, Sally in the Woods and the London Road in Bath and will make limited stops in Farleigh Wick.

The service follows a new partnership between Wiltshire Council and First Bus and will be around 15 minutes quicker than the equivalent journey on the current D1 bus at peak times.

Cllr Caroline Thomas, Wiltshire Council’s cabinet member for transport, said: “We’re delighted to work with First Bus on this new service, which will provide more capacity and faster journeys between Trowbridge, Bradford on Avon and Bath.

“This new service, which we have supported through our Bus Service Improvement Plan (BSIP) funding, meets our Business Plan commitment to have well-connected communities, and at just £2 per journey until the end of this year, the D1x is the perfect bus service for work commutes, shopping trips and leisure journeys.”

George Burton, First West of England’s head of network, said: “Thanks to our work with Wiltshire Council, we’re pleased to be offering the D1x as a quicker route between Trowbridge and Bath, shaving about 15 minutes off the journey time."

The new service will use the new 'tap on, tap-off' technology and will run between existing D1 journeys so passengers will benefit from both faster journeys and an increased frequency of buses between Bath, Bradford on Avon and Trowbridge.

It will have more space for passengers too because it will not pass under the low railway bridge at Limpley Stoke and will be able to use double decker vehicles.

The new D1x will run hourly from around 5am until 7pm Monday to Saturday, with the first trip into Bath leaving Trowbridge at 5.55am and the last trip out of Bath at 6.35pm.

Chippenham MP Michelle Donelan said: “Great news for our community! First Bus and Our Wiltshire are introducing the brand new D1X bus service.

“While neighbouring councils are axing bus routes - Our Wiltshire, with its balanced budget, are able to invest in new bus routes.”

Cllr Johnny Kidney, the Wiltshire Council ward member for Winsley and Westwood, also welcomed the new service, adding: “Unfortunately we won't currently see increased frequency on the D1 timetable at Winsley and Limpley Stoke, however the reduction in overcrowding on this important and popular service will at least be welcome.

“It will also be of benefit to residents in Monkton Farleigh who will be able to use the new bus to access Bath via stops at Farleigh Wick and at the turn-off to Monkton Farleigh.

“I have been assured by the passenger transport team that existing D1 services are otherwise largely unaffected apart from minor timing tweaks, primarily to early morning services and that there will no adverse impact on frequency.”

He added: “While neighbouring Councils like B&NES and Somerset are axing bus routes to try to bridge their budget shortfalls, it is good that Wiltshire Council with its balanced budget is able to buck the trend and invest in new bus routes.

“Alongside this, I will continue to make the case to our passenger transport team at Wiltshire Council to work with First Bus to also increase frequency on the D1 service as bus user numbers continue to rise to pre-pandemic levels.

“I will share the D1 and D1X timetables as soon as they become available."

Hamburg, 2024-02-12. Cotton made in Africa (CmiA) is thrilled to announce its first partnership in the cosmetics sector, with Avon. Avon is part of Brazil-based Natura &Co, one of the world’s largest cosmetics groups. As part of Avon’s ambitious sustainability commitments, it has pledged to use more renewable materials in its supply chains and improve the impact of its products on people and planet. This collaboration with CmiA marks a major step in terms of cotton sustainability for Avon.

“In addition to using sustainably produced raw materials, we aim to achieve certification and traceability and to address both environmental and social aspects for our products containing cotton,” states Natalie Deacon, Avon’s global sustainability head, continuing, “For instance, we have set ourselves the target of using only cotton that is traceable, by 2025, and certified, by 2030. CmiA is the ideal partner to support us in pursuit of this goal, which also contributes to a better world for women, as women play a vital part in CmiA’s work in Africa.” The CmiA standards set out clear criteria regarding gender equality and women’s empowerment, encouraging the non-discriminatory provision of resources, services, and opportunities. This includes pay and benefits, terms and conditions of employment, complaint mechanisms, and flexibility regarding work, training, and other opportunities available to women at the farm and ginnery levels. “We are happy to welcome Avon as our first partner from the cosmetics sector,” says Tina Stridde, the managing director of the Aid by Trade Foundation, adding, “This partnership helps us to expand training and projects for climate resilience and to further extend the social and environmental advantages of sustainable cotton cultivation for small-scale farmers in Africa and their families.”

As a new purchaser of CmiA-certified cotton for products including women’s nightwear, Avon can rely on CmiA’s Hard Identity Preserved (HIP) system, which ensures full transparency and traceability for raw materials all throughout the textile value chain, from the field to the final product. Avon also benefits from this partnership in that CmiA cotton has been proven to have a smaller environmental footprint than other cotton; it is produced without irrigation or genetically modified seeds. Revenue generated by selling licences for sustainably produced CmiA cotton flows back to small-scale farmers, including women, in keeping with AbTF’s social business approach. This involves reinvesting the revenue as funding for training in regenerative cultivation methods and in business fundamentals and for improving biodiversity. In addition to reducing the environmental footprint of cotton production, CmiA promotes human rights and non-discrimination for women and fosters equal access and control over resources for men and women. CmiA thus improves the overall living circumstances of the small-scale farming families. [1]

There are currently around 900,000 small-scale farmers cultivating CmiA-certified cotton in ten countries in Africa South of the Sahara. Consumers can identify these products through their Cotton made in Africa labels. The first products will become available in the second quarter of 2024.
